Cardiovascular disease, and in particular, coronary heart disease (CHD) is the leading
cause of death in the United States for Americans of both sexes and of all racial and ethnic
backgrounds [1–5]. African Americans have the highest overall CHD mortality rate and the
highest out-of-hospital coronary death rate of any ethnic group in the United States,
particularly at younger ages (Box 1)[6–10]. In addition, compared with whites, African
