An 83-year-old woman was referred and admitted to our hospital for treatment of acute
decompensated heart failure. Paroxysmal atrial fibrillation and moderate aortic stenosis had
been detected four years previously. She had no history of diabetes mellitus, dyslipidemia,
or hepatic disorder. She had experienced exertional dyspnea for a week, which was
refractory to oral diuretics. Laboratory data on admission showed high plasma brain
