The last 2 generations of physicians and researchers in pulmonary hypertension (PH) have
undergone their training, performed investigations, and practiced without a thorough
knowledge of the scope of the pathologic alterations underlying PH. Many reasons have
contributed to relegating pulmonary vascular pathology to a secondary role in the overall
clinical management and classification of the disease. Pathology is not part of the clinical
algorithms outlined for diagnosis and
